Youth Transition Coordinator

Provide and coordinate Independent Living Services to youth transitioning from high school, a facility or an institution to live independently within the community of their choice and empower those individuals to reach self-directed goals. Assist youth in individual and group settings, both at the Center and in the individuals’ home and community with person-centered plans, connect youth to community resources, and build self-advocacy skills.  Work closely with schools, families, and agencies who are involved in helping the youth with decision-making, goals and developing independent living skills.  Submit resume to HR at susans@skilonline.com by May 8th.
